1. Senior House Officer
MBBS degree & detail marks sheets of all professional examinations. PM&DCIPMC valid permanent registration certificate. Copy of publications (if any) in an approved journal from PM&DC/PMC/PubMed Indexed & HEC. Domicile Certificate. Computerized National Identity Card (CNIC). Relevant subject/specialty related experience certificate from Public Sector & any Private Teaching Hospital recognized by PMC for Postgraduation Training. One passport-size photograph duly attested from the backside. Those having foreign qualifications are required to submit HEC/PMDC/PMC equivalency certificate.

Copy of Inter Branch Transaction must be attached with the application form. Forms are also available from the account section of FPGMI, Lahore on Payment of Rs. 20001-. Applicants cannot apply to more than 02 departments. The information given in the application form shall be verified at three (03) stages: – During the submission & processing of the application.
