PM&DC/PMC valid permanent registration certificate. House Job certificate with exact dates from the concerned Medical Superintendent/Administrators. Domicile Certificate. Computerized National Identity Card (CNIC). Three passport size photographs duly attested from the backside. Those having foreign qualifications are required to submit an HEC / PMDC / PMC equivalency certificate.
