4. FCPS Part-I Pass Certificate
5. CNIC Copy
6. PMDC Certificate
7. Domicile (Open) (preferred GB Domicile).
8. No Objection Certificate (N.O.C) from the competent authority for regular and contractual Job doctors. No application will be received without N.O.C.
9. 03 passport-size color photographs.
